Claim Reporting Instructions

How to Submit a Claim Report

We want you to have the most positive claims handling experience possible. There are several ways to report claims. Below you will find reporting instructions by policy type and carrier. Reporting Instructions by Policy Type

Select your policy type to view reporting instructions for the following:

  • Package Policies
    For Package Policies Written Through Granite State Insurance Company, New Hampshire Insurance Company, Illinois National Insurance Company, Lexington Insurance Company, National Union
  • Directors & Officers
    Loss reporting for Directors & Officers (D&O) policies written with USLI®, CHARTIS®, or Zurich® North America.
  • Specialty, Monoline & Pollution
    Specialty, monoline and pollution policies written with CHARTIS®, Lexington Insurance and others.
  • CNA Insurance Company
    ISA Policies older than 5 years old

Report a Loss Online (ISA Insureds Only)

We offer online claims reporting for Property, Auto and General/Professional Liability for ISA insureds only. When you complete the online form, it is submitted directly to Irwin Siegel Agency, Inc. If the form is submitted outside business hours (Mon.-Fri. 8am – 4:30pm) or on a weekend, it will be sent to the carrier for processing the next business day.
