Once we receive your information, one of our experienced underwriters will call you. The underwriter will give you the status of your submission and answer any additional questions you may have.

Prequalify a Risk
There are several ways you can prequalify a risk:
- Online: Fill out this form to prequalify a risk online.
- E-mail/fax: Send a brochure/summary of the risk’s services to us for pre-qualification at siegel@siegelagency.com or fax to 845-796-3661.

Download Applications
Fill out the General Application and supplemental application that best fits the risk. Download program-specific applications from our website by visiting our Programs section or visit the Application Index for quick links to applications.
What to Include
Include the following information along with your applications:
- Brochure or website URL
- Application (ISA will work off another carrier’s application or our own)
- Five (5) years currently valued company loss runs with expiring premiums
- ACORDs or coverage requirements
- Drivers list
- Audited financials or 1120 income tax returns (if for-profit)
Additional Requirements for Start-Ups:
- Financials or Pro-Forma Statement
- Resume for Executive Director (showing 3-5 years experience)
- Business Plan
